Clinical Nurse Leader (CNL) Sequence

(22  semester hours plus 12 hours core)

Graduates are prepared to function as leaders within complex health care environments. The practice of the CNL emphasizes nursing leadership, client outcomes management, and care environment management. The CNL is prepared as a generalist and functions at the microsystem level as provider and manager of care at the point of care delivery. Design, implementation, and evaluation of research based care; coordination, delegation, and supervision of care provided by the health care team are essential functions of the role.

 

In addition to the 12 hour core, there are 9 hours of support courses and 13 hours of specialty courses for the Clinical Nurse Leader Sequence. The support courses are NUR 431, 437, and 479. The specialty courses are NUR 432, 491, and 492.

  7. Description of Proposed Program or Name Change: The clinical nurse leader (CNL) sequence is an additional educational sequence in the Master of Science in Nursing program. The practice of the CNL emphasizes nursing leadership, client outcomes management, and care environment management. The CNL is prepared as a generalist and is not eligible for advanced practice license.

 

 “The CNL is a leader in the health care delivery system across all settings in which health care is delivered. The CNL functions within a microsystem and assumes accountability for healthcare outcomes for a specific group of clients within a unit or setting through the assimilation and application of research-based information to design, implement, and evaluate client plans of care. The CNL is a provider and a manager of care at the point of care to individuals and cohorts. The CNL designs, implements, and evaluates client care by coordinating, delegating and supervising the care provided by the health care team, including licensed nurses, technicians, and other health professionals” (American Association of Colleges of Nursing, 2004).

 

  1. Rationale for Proposal:

A Major Objective for Mennonite College of Nursing in FY06 follows:

4. Increase and enhance collaborative partnerships that will allow the college to pursue innovations in nursing education.

 

This Major Objective is in alignment with Educating Illinois Goal 7. The partner fully engaged in outreach with business, industry, government, and education.

 

Mennonite College of Nursing is a recognized leader in developing innovative nursing education partnerships. The development of the Clinical Nurse Leader Sequence represents an active partnership among Mennonite College of Nursing, the American Association of Colleges of Nursing and 3 central Illinois medical centers to develop, implement, and evaluate the education and practice experiences of the Clinical Nurse Leader registered nurse. Providing safe and competent care for patients in complex health care environments remains a challenge for health care systems. The shortage of registered nurses to provide care is only one challenge facing the achievement of safe and effective care. Mounting evidence indicates that educational preparation of nurses is considered to be of equal importance in the delivery of safe and effective patient care.

 

The Clinical Nurse Leader registered nurse would not duplicate the role of the current bedside registered nurse or advanced practice nurse, but rather provide a new type of care provider with additional skills and research knowledge to impact patient safety, outcomes and the structure and processes within the health care system. The CNL provides evidence-based patient care, continuity of care, and optimizes existing staff’s ability to provide high quality care in a cost efficient manner.

 

Community medical centers and clinical partners engaged with Mennonite College of Nursing include, BroMenn Medical Center; OSF St. Joseph’s Medical Center; Memorial Medical Center. These 3 acute care institutions in central Illinois have joined MCN in a partnership initiative to evaluate the impact of a new type of health care provider, the Clinical Nurse Leader (CNL). In addition all institutions have committed to providing clinical practice sites and stipends for students during the intensive summer semester educational experience. All clinical partners are working with MCN to develop position descriptions for new CNL graduates to secure positions in their facility.

  1. Expected Impact of Proposal on Existing Campus Programs: The CNL sequence impacts the MCN graduate program only. Offering the CNL sequence will increase the total number graduate nursing students at MCN.
  2. Curricular Change Including New Courses: The proposed sequence is based upon the established core curriculum of the Nursing Master’s program and the curricular guidelines and core competencies requirements established by the American Association of Colleges of Nursing. The CNL sequence requires the addition 3 new courses not currently offered in the Master’s program. Students enrolled in the CNL sequence complete 22 hours of CNL sequence courses plus 12 hours core of the Master’s program.

CNL sequence courses include:

·         NUR 437: Pathophysiologic Bases of Health Deviation 3 sem. hrs. (existing course)

·         NUR 431: Diagnostic Reasoning for Advanced Nursing Practice 3 sem. hrs. (existing course)

·         NUR 479: Advanced Concepts for Nurse Leaders 3 sem. hrs. (existing course)

·         NUR 432: Pharmacotherapeutics for the Clinical Nurse Leader (online) 2 sem hrs (NEW Course)

·         NUR 491: Clinical Nurse Leader I: Care Environment Management 4 sem hrs (NEW Course)

·         NUR 492: Clinical Nurse Leader II: Practicum 7 sem hrs (NEW Course)

 

Core courses (all courses are existing courses required in other Master’s sequences):

·         NUR 401: Theoretical Foundations of Nursing and Health Care, 3 sem. hrs.

·         NUR 403: Professional Roles and Issues, 3 sem. hrs.

·         NUR 405: Epidemiology and Aggregate Based Health Promotion, 3 sem. hrs

·         NUR 407: Scientific Inquiry and Research Design in Health Care, 3 sem. hrs)

  1. Anticipated Staffing Arrangements: No new staffing arrangements are needed with the approval of the CNL sequence.
  2. Anticipated Funding Needs and Source of Funds: Clinical partners have committed to payment of stipends to students during the intensive summer clinical session. Summer salary will be needed for one faculty member teaching the summer session. The Dean of MCN is working with college development staff to seek additional funding to support summer faculty salary.

No new resources are requested nor will reallocation from other departments occur. The courses for the CNL are at the Master’s level and 21 hours of the 34 hours curricular hours are currently being offered in the graduate program. The CNL sequence will encourage more efficient use of resources because it will generate greater enrollment in the master’s program existing core and support courses.
